MINE FATALITY FINAL REPORT – On June 24, 2019, a contractor fell beneath the wheels of a tractor-trailer that was stuck in the sand. The contractor was walking toward the side of the truck as it was being extracted with a bulldozer. He fell and died at the scene.

Best Practices:

  • Confirm your visibility. Do not approach large mobile equipment, until you communicate and verify with the equipment operator your planned movement and location.
  • Stay in the line of sight with mobile equipment operators. Never assume the equipment operator sees you.
  • Confirm a clear path before moving equipment. Use words or signals.
  • Buckle up. Do not allow people to ride in any area of a vehicle that is not equipped with a seat belt.
